New Students' Registration

Download and complete the general registration packet as well as the appropriate grade level packet located in the Forms section of this page. New students' registration packets must include the following:

  1. Orders
  2. Area Clearance
  3. Passport or Birth Certificate
  4. Immunization Records (see the DoDEA requirements)
  5. Recent Physical. Essential if the student wishes to play sports.
  6. Previous school records. Official transcripts/last report card or withdrawal slip from previous school. If known, provide address and fax number of previous school. This includes paperwork that indicates any special services (ESL, Gifted and Talented, Individualized Education Plan, etc).
  7. Student Social Security Number. If the student does not have a social security number, provide ID number from Social Security Office.
  8. The sponsor's signature is required on forms for initial registration. A Power of Attorney is acceptable. To simplify the registration process it is a good idea to contact the school and request the necessary paperwork. If you have this material prior to arrival, the registration process will be more expedient.

Re-Registration

Each May, parents must re-register students for the following school year. Re-registration consists of verifying or correcting the information on the current registration form. The sponsor's signature is NOT required for re-registration. Re-registration also includes a stop in the nurse's office. We ask you to review your child's health record for any changes in their health condition. (For example, newly prescribed medications that will be administered on a daily basis, new glasses, allergies, asthma, etc.) Your child's immunization records are also reviewed and updated at that time.

Transfer/Withdrawal of Students

The school office and the classroom teacher should be notified of the imminent departure of students from Iwakuni as soon as the sponsor receives orders. Parents must come to the main office and fill out a Withdrawal/Records Request form at least two weeks prior to the last day of attendance. A copy of the sponsors orders must be submitted with the request. If the child is leaving due to Early Return of Dependents, then a letter from the command must be submitted. Copies of the child's record will be available for the parent to pick up at the end of the school day on the last day of attendance.
